    This page allows you to select a form and send email(s) to members of a specific distribution list.

    Title - The name of the new form.

    System Template - Select from any of the available templates:

  • Attendance Tracker For Events - Application that used Registration data from
  • Contract Registration Form - Name and email address fields automatically added. Additional questions can be added but the system generated questions will be processed as contact information when the form is submitted.
  • Custom Form - The email address question is added by default. Additional fields can be added but the system generated email question allows the email address to be added to a distribution list.
  • Event Registration Form - The event name, date/time, and location questions are added by default. Additional questions can be added as required.
  • Custom Template - If you have created one or more form templates, they will appear in this dropdown list.
