This page displays all the forms that have been created.

Title - The name given to the form upon creation. It can also be changed at a later time in the 'Form Editor' page.

Allows new surveys to be created.

Edit the selected survey.

Launch the survey to allow data to be entered and submitted.

Download the responses that have been submitted for the form.

Delete the survey and all responses for the survey.

The survey is currently offline. Click this icon to make the survey active and available for display and submission.

The survey is currently active. Click this icon to make the survey offline where is cannot be displayed.
